Organ and Eye Bank Guidelines for Area Lion Clubs January 21, 2007     The Organ and Eye Bank will grant one free voucher per month to each participating Lion Club in good standing. Vouchers may be surrendered at the Adult Vision Clinic located at 130 N. Central Expressway McKinney, TX 75070. 972-540-9046   This free monthly voucher will entitled each club to: One free eye examination ($55.00 value).       Glasses prices are as follows:         Single Vision Glasses (Frame/Lenses/Dispensed, $48.00 )                                        Or Bifocal Glasses (Frame/Flat Top 28 Lenses/Dispensed, $73.00)   The 2E2 Organ and Eye Bank will bill each respective Lion’s Club for each eyeglass charge.    ________________________________________________________________________   The Organ and Eye Bank has secured a national billing account enabling all Lions Clubs to purchase eyewear. In conjunction with the voucher system, all clubs will have access to a national opticianry service with full warranted eyewear.       The Voucher System can save on examination costs anywhere from $38 to $134.00 per examination.

Additional Voucher Purchases –    In addition to the initial free monthly voucher, Lion Clubs can purchase additional services by contributing to a yearly donation plan at the level of their choice.   Lions Organ & Eye Bank Voucher Level Programs.                    Level One - $50 per month or $600/year – 36 additional vouchers (3 per month)   Level Two - $100 per month or $1200/year – 60 additional vouchers (5 per month)   Level Three - $150 per month or $1800/year – 84 additional vouchers (7 per month)    Voucher Club Savings on Vision Examinations    (Average eye examination in N.
